Racism in health and healthcare has long been recognised as a structural issue. While there has been growing research and a number of important initiatives that have come from approaching racism as a structural issue, there is a range of implications that yet have to be explored as they relate to health and healthcare. Conceptualising racism in this way provides a means to consider how it shapes and is shaped by a range of global injustices and serves as a foundation (...) for more egregious harms. It also suggests that if we are to dismantle racism, we need to look both within and beyond the traditional domains of health and healthcare and account for a range of broader forces that sustain and re‐enforce racism. We first discuss the issue of responsibility, drawing on Young's social connection model to argue that we all have a responsibility to take action in addressing structural racism. We will then deal with a question that naturally follows, namely how we discharge our responsibilities, with a focus on the role of disruptive action in challenging power and ignorance in dismantling racism in health and healthcare. Background Ethical climate refers to the shared perception of ethical norms and sets the scope for what is ethical and acceptable behaviour within teams. Aim This paper sought to explore perceptions of ethical climate amongst healthcare workers as measured by the Ethical Climate Questionnaire (ECQ), the Hospital Ethical Climate Survey (HECS) and the Ethics Environment Questionnaire (EEQ). Methods A systematic review and meta-analysis was utilised. PSYCINFO, CINAHL, WEB OF SCIENCE, MEDLINE and EMBASE were searched, and papers were included if they (...) sampled healthcare workers and used the ECQ, HECS or EEQ. Ethical consideration Ethical approval was not required. Results The search returned 1020 results. After screening, 61 papers were included ( n = 43 HECS, n = 15 ECQ, n = 3 EEQ). The overall sample size was over 17,000. The pooled mean score for the HECS was 3.60. Mean scores of individual studies ranged from 2.97 to 4.5. For the HECS studies, meta-regression was carried out. No relationship was found between the country of the studies, the study setting (ICU v non-ICU settings) or the mean years of experience that the sample had. For the ECQ, sub-scales had mean scores ranging from 3.41 (instrumental) to 4.34 (law) and were all observed to have significant and substantial heterogeneity. Three studies utilised the EEQ so further analysis was not carried out. Conclusions The above results provide insight into the variability of scores as measured by the HECS, ECQ and EEQ. To some extent, this variability is not surprising with studies carried out across 21 countries and in a range of healthcare systems. Results also suggest that it may be that more local and context specific factors are more important when it comes to predicting ethical climate. (shrink)

Importance Informed consent is essential to ethical, rigorous research and is important to recruitment and retention in cancer trials.Objective To examine cancer clinical trial (CCT) participants’ perceptions of informed consent processes and variations in perceptions by cancer type.Design and Setting and Participants Cross-sectional survey from mixed-methods study at National Cancer Institute–designated Northeast comprehensive cancer center. Open-ended and forced-choice items addressed: (1) enrollment and informed consent experiences and (2) decision-making processes, including risk-benefit assessment. Eligibility: CCT participant with gastro-intestinal or genitourinary, hematologic-lymphatic (...) malignancies, lung cancer, and breast or gynecological cancer (N = 334).Main Outcome Measures Percentages satisfied with consent process and information provided; and assessing participation’s perceptions of risks/benefits. Multivariable logistic or ordinal regression examined differences by cancer type.Results Most patient-participants felt well informed by the consent process (more than 90% overall and by cancer type) and. most (87.4%) reported that the consent form provided all the information they wanted, although nearly half (44.8%) reported that they read the form somewhat carefully or less. More than half (57.9%) said that talking to research staff (i.e., the consent process) had a greater impact on participation decisions than reading the consent form (2.1%). A third (31.1%) were very sure of joining in research studies before the informed consent process (almost half of lung cancer patients did—47.1%). Most patients personally assessed the risks and benefits before consenting. However, trust in physicians played an important role in the decision to enroll in CCT.Conclusions and Relevance Cancer patients rely less on written features of the informed consent process than on information obtained from the research staff and their own physicians. Research should focus on information and communication strategies that support informed consent from referring physicians, researchers, and others to improve patient risk-benefit assessment and decision-making. Background Informed consent is often cited as the “cornerstone” of research ethics. Its intent is that participants enter research voluntarily, with an understanding of what their participation entails. Despite agreement on the necessity to obtain informed consent in research, opinions vary on the threshold of disclosure necessary and the best method to obtain consent. We aimed to investigate Australian researchers’ views on, and their experiences with, obtaining informed consent. Methods Semi-structured interviews were conducted with 23 researchers from NSW institutions, working (...) in various fields of research. Interviews were analysed and coded to identify themes. Results Researchers reported that consent involved information disclosure, understanding and a voluntary decision. They emphasised the variability of consent interactions, which were dependent on potential participants’ abilities and interests, study complexity and context. All researchers reported providing written information to potential participants, yet questioned the readability and utility of this information. The majority reported using signed consent forms to ‘operationalise’ consent and reported little awareness of, and lack of support in implementing more dynamic informed consent procedures, such as verbal informed consent, that was fit for the purposes of their studies. Views on Human Research Ethics Committees varied. Some reported inconsistent, arduous inputs on the information form and consent process. Others expressed reliance on HRECs for guidance, viewing them as institutional safeguards. Conclusions This study highlights the importance of transparent relationships, both between researchers and participants, and between researchers and HRECs. Where the relationship with study participants was reported as more robust, researchers felt that they were better able to ensure participants made better, more informed decisions. Where the relationship with HRECs was reported as more robust, researchers were more likely to view them as institutional safeguards, rather than as bureaucratic hindrances. Conscientious and mindful researchers are paramount to ensuring the procedure accommodates individual requirements. This study advocates that when designing ethical informed consent practices, researchers should be integrated as autonomous players with a positive input on the process, rather than, in the worst case, predatory recruiters to be curtailed by information forms and oversight. In this article, I raise a simple but surprisingly vexing question: What makes heavy metal heavy? We commonly describe music as “heavy,” whether as criticism or praise. But what does “heavy” mean? How is it applied as an aesthetic term? Drawing on sociological and musicological studies of heavy metal, as well as recent work on the aesthetics of rock music, I discuss the relevant musical properties of heaviness. The modest aim of this article, however, is to show the difficulty, if (...) not impossibility, of this seemingly straightforward task. I first address the difficulties of identifying the defining features, or “Gestalt,” of heavy metal that would allow us to treat heaviness as a genre concept. Next, I discuss both the merits and the limits of analyzing heaviness in terms of an aesthetics of “noise” in rock music developed in recent philosophy of music. In the remaining sections, I consider other nonaesthetic features relevant to aesthetic judgments of heaviness and show that the term ‘heavy’ is conceptually inarticulable, if not irreducible. This, I conclude, has partly to do with the radically different, sometimes incompatible, musical properties present in the perception of musical heaviness. (shrink)

In this journal S Siddharth has recently argued that the phenomenal bonding response to the subject summing argument for panpsychism is question begging, therefore we should reject constitutive forms of panpsychism. The argument specifically focuses on the proposals of Goff and Miller. In this reply, I show that the argument is unsound.

In both politics and art in recent decades, there has been a dramatic shift in emphasis on representation of identity. Liberal ideals of universality and individuality have given way to a concern with the visibility and recognition of underrepresented groups. Modernist and postmodernist celebrations of disruption and subversion have been challenged by the view that representation is integral to social change. Despite this convergence, neither political nor aesthetic theory has given much attention to the increasingly central role of art in (...) debates and struggles over cultural identity in the public sphere. -/- Connecting Hegelian aesthetics with contemporary cultural politics, Jason Miller argues that both the aesthetic and political value of art are found in the reflexive self-awareness that artistic representation enables. The significance of art in modern life is that it shows us both the particular element in humanity as well as the human element in particularity. Just as Hegel asks us to acknowledge how different historical and cultural contexts produce radically different experiences of art, identity-based art calls on its audiences to situate themselves in relation to perspectives and experiences potentially quite remote—or even inaccessible—from their own. Miller offers a timely response to questions such as: How does contemporary art’s politics of perception contest liberal notions of deliberative politics? How does the cultural identity of the artist relate to the representations of cultural identity in their work? How do we understand and evaluate identity-based art aesthetically? -/- Discussing a wide range of works of art and popular culture—from Antigone to Do the Right Thing and The Wire—this book develops a new conceptual framework for understanding the representation of cultural identity that affirms art’s capacity to effect social change. An increasingly popular theory of moral responsibility, Attributionism, identifies attitudes as the locus of direct responsibility. And yet, two agents with qualitatively identical attitudes may differ in their responsibility due to a difference in whether they act on those attitudes. On the most plausible interpretation of Attributionism, attitude duplicates differ in their responsibility only with respect to the scope of what they’re responsible for: one agent is responsible for only their attitudes, while the other is responsible for their attitudes and (...) for acting in a way that reflects those attitudes. Against this, I argue that attitude duplicates may also differ with respect to their degrees of praiseworthiness, and that this is best explained by either the effort or sacrifice instantiated in one’s actions—explanations unavailable to Attributionism. If this is correct, then Attributionism fails to provide an adequate account of praiseworthiness, and therefore fails as a theory of moral responsibility. (shrink)

According to the No Alternate Possibilities argument, if time passes then the rate at which it passes could be different. Thus, time cannot pass, since if time passes, then necessarily it passes at a rate of 1 second per second. One response to this argument is to posit hypertime, and to argue that at different worlds, time passes at different rates when measured against hypertime. Since many A-theorists think we can make sense of temporal passage without positing hypertime, we pursue (...) a different response. We describe several worlds that do not contain hypertime, but do contain differential passage: worlds where time passes at different rates in different subregions within the same world. Hence we argue that even if we focus just on the set of worlds that do not contain hypertime (whether this is all, or only some of the worlds) we find that not all these worlds are such that time passes at a rate of 1 second per second. Thus the No Alternate Possibilities argument fails even when restricted to just this set of worlds. (shrink)

The GRW dynamics propose a novel, relevantly “observer”-independent replacement for orthodox “measurement”-induced collapse. Yet the tails problem shows that this dynamical innovation is not enough: a principled alternative to the orthodox account demands some corresponding ontological advancement as well. In fact, there are three rival fundamental ontologies on offer for the GRW dynamics. Debate about the relative merits of these candidates is a microcosm of broader disagreement about the role of ontology in our physical theorizing. According to imprimitivists, the GRW (...) dynamics directly describe (only) some (element’s) undulation in an unfamiliar high-dimensional physical field. Primitivists resist this GRW0 proposal on the grounds that it fails to secure comprehensible contact with our data about macroscopic objects in ordinary lowdimensional space-time. They expect an adequate fundamental ontology to include at least some spatiotemporally localized entities—intuitively, concrete constituents of our familiar macroscopic landscape. The most compelling case goes by way of distributional basing: minimally, primitivists expect a theory’s predictions immediately about spatiotemporal distributions of fundamental entities to provide a supervenience base for data about configurations of macroscopic objects. But while the background intuition is familiar, the distributional model is surprisingly subtle. Lack of clarity about its details generates serious confusion for both sides of our debate. (shrink)

Recent incidences of fraud and the growing globalization of business have focused attention on the effect of culture on ethical decision making within organizations. Because fraud can be extremely costly and is more likely to be committed by employees than persons external to the organization, employees willing to report un-ethical acts are an important supplemental control tool. The current study provides evidence of the effects of culture (Canadian and Chinese) and the type of reporting (whistle-blowing and peer reporting) on reporting (...) unethical acts within organizations. Chinese and Canadian participants were asked to respond to scenarios describing unethical acts committed within organizations. Results indicate that Chinese are more likely to report the unethical acts of peers than Canadians. Chinese also appear more likely to report the unethical acts of peers than supervisors. The results also suggest that Graham’s model of principled organizational dissent may not be applicable to Chinese and perhaps other non-Western cultures. (shrink)

Although it is frequently overlooked, J.S. Mill's political philosophy has a significant civic component; he is a committed believer in the value of active and disinterested participation in public affairs by the citizens of liberal democracies, and he advocates a programme of civic education intended to cultivate public spirit. In the first half of this essay I present a brief but systematic exploration of his thought's civic dimension. In the second half I defend Mill's civic liberalism against various critics who (...) have explicitly or implicitly charged that the civic and liberal components of his political philosophy are inconsistent. Support vector machines (SVMs) are being used increasingly in affective science as a data-driven classification method and feature reduction technique. Whereas traditional statistical methods typically compare group averages on selected variables, SVMs use a predictive algorithm to learn multivariate patterns that optimally discriminate between groups. In this review, we provide a framework for understanding the methods of SVM-based analyses and summarize the findings of seminal studies that use SVMs for classification or data reduction in the behavioral and neural study of (...) emotion and affective disorders. We conclude by discussing promising directions and potential applications of SVMs in future research in affective science. (shrink)

This paper presents the treatment of anaphora in generative grammar. After a broad review of of the different theories covered under the term, it provides an overview of the history of the treatment of anaphora in generative grammar. It then discusses Ariel’s theory of accessibility and shows how it can account for the choice of some of the anaphoric expressions provided in the text. In a final section, the paper presents the author’s recent work on the choice between verbal anaphors. (...) It is shown that accessibility can account for some aspects of the choice but is not sufficient. Further constraints on the choice are proposed in terms of the notion of Question Under Discussion, for which experimental evidence is adduced. (shrink)

I argue that Schmidtet al, while correctly diagnosing the serious racial inequity in current ventilator rationing procedures, misidentify a corresponding racial inequity issue in alternative ‘unweighted lottery’ procedures. Unweighted lottery procedures do not ‘compound’ (in the relevant sense) prior structural injustices. However, Schmidtet aldo gesture towards a real problem with unweighted lotteries that previous advocates of lottery-based allocation procedures, myself included, have previously overlooked. On the basis that there are independent reasons to prefer lottery-based allocation of scarce lifesaving healthcare resources, (...) I develop this idea, arguing that unweighted lottery procedures fail to satisfy healthcare providers’ duty to prevent unjust population-level health outcomes, and thus that lotteries weighted in favour of Black individuals (and others who experience serious health injustice) are to be preferred. I argue that Schaffer fails to provide a non-question-begging argument for priority monism. Despite his suggestion to the contrary, Humean pluralists need not, and plausibly do not, endorse his tiling constraint on metaphysically basic objects. Moreover, the distinction between supervenience—of the sort at issue in Humean doctrine—and metaphysical necessitation—of the sort at issue in Schaffer’s tiling constraint—points toward an alternative treatment of the phenomena initially inspiring Schafferian monism. There is an important possibility, one that Humeans can or should embrace, that (...) Schaffer overlooks when drawing his monistic conclusion. (shrink)
